A comprehensive international survey has revealed that women are twice as likely as men to receive electroshock therapy (ECT) and experience more negative effects. The study, led by Professor John Read from the University of East London, surveyed 858 ECT recipients from 44 countries, 73% of whom were women. These results highlight systemic gender inequalities in mental health care and call for a trauma-sensitive reevaluation of ECT practice.
Women reported significantly higher rates of long-term memory loss, short-term memory problems, and traumatic reactions. Only 15% of women would choose ECT again, compared to 29% of men. This discrepancy suggests that women benefit less from treatment and are more likely to have negative experiences. The survey also shows that women are given less information before treatment and are put under more pressure to give consent.
The results of the study underline the need for a trauma-sensitive, feminist perspective in psychiatric care. The experiences of women who describe ECT as retraumatizing are alarming. Many reported feelings of hurt or loss of control that brought back memories of past abuse. These reports highlight that for some women, ECT is a form of psychological violence.
Professor Read calls for an urgent reassessment of ECT use, particularly in relation to gender differences in risk and outcome. Information about these differences needs to be routinely communicated to women and their families. ECT should not be the standard answer to women’s suffering, but rather comprehensive education about the risks and alternatives is needed.
