A possible case of infant botulism has been reported in Pierce County, Washington, which may be linked to a national outbreak. This outbreak is associated with the recalled ByHeart Whole Nutrition Infant Formula. Tacoma-Pierce County health officials said the affected child was diagnosed Thursday and is currently being treated at a local hospital.
Infants are particularly susceptible to botulism because their gut microbiome is not yet sufficiently developed to prevent the spores from germinating and producing the toxin. Symptoms can include muscle weakness, difficulty swallowing and breathing problems, often requiring ventilation. Since August 2025, at least 23 cases have been identified in 13 states, including one case in Lewis County.
The US Food and Drug Administration (FDA) is currently conducting an investigation to determine the point of contamination. FDA inspectors visited ByHeart’s manufacturing facilities in Allerton, Iowa, and Portland, Oregon, this week. ByHeart products account for about 1% of the total infant formula market sold in the U.S., and the FDA says there are no concerns about infant formula shortages.
Although infant botulism can be potentially fatal, no deaths have been reported in this outbreak to date. However, several infants across the country remain hospitalized. The affected ByHeart baby formula was sold through its website and at retailers including Amazon, Kroger, Walmart, Whole Foods and Target, and was widely available in Washington.
Parents who have recently purchased ByHeart Whole Nutrition Infant Formula are advised not to use it and to either discard it or return it to the place of purchase. It is important to note the affected lot numbers before discarding the containers. Surfaces that have come into contact with the recalled food should be thoroughly cleaned. If symptoms such as poor feeding or loss of head control occur, medical attention should be sought immediately.
